Attachments to the Controlled Substance Policy - How and why you must comply

Below you will find a brief description of each form.
Power of Attorney Form (POA): This form is to be used by the DEA Registrant to allow an employee to act as a proxy for the registrant as it pertains to Controlled Substances. This form should be filed at the practice with controlled substance records.
Power of Attorney Revocation Form: This form is used to revoke the Power of Attorney if needed. This form should be filed at the practice with controlled substance records.
Screening Questionnaire: This form is required by the DEA. The registrant has to screen employees that have access to controlled substances. This form should also be maintained at the practice with employee files.
Rx Reverse Distributor (Destruction Agreement): This form should be used to process expired controlled substances so that they are disposed of properly. The form attached is used to set up an account with the reverse distributor.
