How to Complete the ACH Vendor Form

This article provides guidance on how to complete the ACH Vendor Form (see attachment) that Veterinary Student Externs need to complete in order to receive their stipend payment. Details on this form sometimes cause them confusion, so this article will help answer some of their frequently asked questions.

Section 1: Payee/Company Information

Complete this section in its entirety. You would list yourself as the Name and also for the Contact Person Name.

Section 2: Financial Institution Information - ACH (Direct Deposit)

This section is what will create the direct deposit of funds.
For Name, please list the name of your bank. For Telephone number and Address, please list the information for a bank branch that you use.
You do NOT need to list an ACH Coordinator Name.
Please review the check diagram below to identify the ACH Bank Routing Number and Account Number.
Please fill out the Name on Account with your name, as it's listed with the bank.
Check the box indicating Checking or Savings, based on the type of account you have.
Please sign there for the box asking for Signature and Title of Authorized Official, but you do NOT need to list a title.

Section 3: Financial Institution Information - Wires

• You would only complete this information if you decided to have a wire transfer rather than the ACH/Direct deposit option above.
